Survey Registration Page
If your organization has already completed the SAY Survey, that information is
in our database and you do not need to retake the survey. You will be contacted
soon about updating your program data.
Since November 2003, more
than 1500 organizations, corporations, and communities, representing every
sector of the US, have provided information about their efforts to reverse the
health crisis of overweight and inactivity in young people. If your
organization or program is involved, directly or indirectly, in these areas, we
urge you to join this ongoing project by completing this survey. Every
organization and every response counts.
If you know of other
organizations that should be included in this initiative, please forward this
registration information to them.
Registering Your Program

Before receiving the
information that you will need to access the survey, you must first register
your program.
|
|
|
You will be asked to provide
general information about your program and for contact information for the
person in your organization to whom the survey should be sent. Registration
takes about 5 minutes. |
|
|
After registering, an email message
will be sent to the contact person you have named with log-in information for
accessing the survey, which takes 20-30 minutes to complete.
|
|
|
If yours is solely a funding
organization, you will be asked general information about your funding policies
during registration, and this will complete your participation. |
|
|
If you would prefer to register and
take the survey by telephone, please call us at 1-800-SAY-9221 or email
survey@academicnetwork.com to schedule an appointment to do so.
