The initial goal of the Shaping America’s Youth initiative is to identify the multitude of individual efforts that are currently underway to reverse the national health crisis of overweight and inactive children and adolescents. A centralized clearinghouse, or registry, of these programs and initiatives will enable and promote interactions, partnerships, and coordinated efforts among all concerned parties to improve the health of America’s children.

Your participation is critical to the success of this initiative. The creation of a comprehensive database that identifies what is being done and how it is being done is the first step towards resolving this health crisis. To achieve this, information must be collected from as many programs and organizations as can be identified. Clearly, the more information that is included in this registry, the more effective a tool it will become.

As a survey participant, you will be given the option to have your program listed in the registry. You will have Internet access to the executive summary of this initiative when it is complete in early spring of next year, and will be notified when this becomes available.

As an active member of the community that is fighting childhood overweight and inactivity, you know the challenges to resolving these conditions. By participating in this survey, you will be participating in the process of finding ways to meet those challenges. You will be contributing to both the overall goals of this national initiative and the objectives of your own work – to promote the health of our kids.
